What we need to know to provide optimal service

This is a three-part session. The history of the recognition of diabetes and its subtypes  and the relationship of increased obesity and other factors that contribute to the dramatic increase in the numbers of children and adults with diabetes will be in Part I. Causal influences of type 1 and 2 in children and adults, current conditions, complexity of care, and adherence risk factors also will be discussed. Part II will consider the functional and cognitive decline in aging and strategies related to medication regimens. Part III will explore psychological issues surrounding diabetes and how mental health practitioners can best emotionally support individuals and their families, including working with inter-disciplinary teams. One nurse CDE, one dietician dietitian CDE , and one psychologist-practitioner will present. PSCP sees the program as the first of several that will look at chronic illness and how psychologists can help.

Lisa Laird, RN, BSN, CDE, will present at AADE National Leadership Meeting in January 2018. As AADE Coordinating Body Chair, she has coordinated and led two successful state-wide annual meetings with multi-discipline healthcare professionals. She created a budget for Pennsylvania, expanding the leadership to include Delaware as a local networking body of the PA AADE and devised and implemented a state-wide marketing campaign. She enjoys most creative problem solving involving individual patients, families, and systems, seeking an ideal model of health care delivery to improve quality of life and the patient experience.

Julie Meranze Levitt, PhD, is a clinical and school psychologist who has treated children and adults who have chronic diseases for over 40 years. She also is a systems psychologist who has studied relationships of individuals with chronic medical problems within family contexts. Dr. Meranze Levitt ‘s doctoral studies in psychology are from Yeshiva University and her post-doctoral training in individual and family therapies were at St. Christopher’s Hospital for Children and Temple University School of Medicine and Philadelphia Child Guidance Center. She is a hypnotherapist and was a training psychologist at the Council of Relationships from 1980-1995.

Kathy Stroh, MS, RD, LDN, CD is a Registered Dietitian and Certified Diabetes Educator (CDE) and has been on the staff of state-level departments for older adults and adults with physical disabilities as well as the state-level Diabetes Prevention and Control Program. She currently is the CDE for a multi-site Federally Qualified Health Center (FQHC). She has been a presenter at state and national conferences for the American Association of Diabetes Educators (AADE), American Diabetes Association and the Centers for Disease Control and Prevention. She is co-author of AADE’s Practice Advisory “Special Considerations in the Management and Education of Older Adults with DM”, and “Individualizing DSMES”.
